The program is designed to be completed in two semesters when taken on a full-time basis. Students should complete the program within the specified timeframe. Entrance into the Phlebotomy Program requires a meeting with the Allied Health Program Director and submission of the required documents listed on the Phlebotomy Program Information packet. Students may elect to complete some or all of the general education course requirements prior to entering the Phlebotomy Program. Continuation in the Phlebotomy Program requires a minimum grade of "C" in MAST/ALTH courses, a minimum grade of "C" in all other required courses and a minimum cumulative and most current completed term grade point average of 2.4 to participate in the practicum experience. There is a time limit on completion of program requirements. Algebra and biology courses should have been completed within the past five years or updating courses is required. Graduates are eligible to earn the Phlebotomy Technician (PBT) credential through the American Society of Clinical Pathology (ASCP) credentialing exam and the Registered Phlebotomy Technician (RPT) credential through the American Medical Technologists (AMT) credentialing exam.
Step 1 - Apply for Admission to Southern State.
Step 2 - Thoroughly read and complete the Phlebotomy Technician Program Information Packet
Enrollment in Allied Health courses is limited. Entrance is granted on a first-come, first-served basis after the above criteria are met. After entry, but before beginning their practicum experience, students will be required to:
- 1. provide medical examination and immunization information indicating they meet the health requirements (have the physical and emotional stamina) to pursue this career for practicum experiences;
- 2. complete a criminal background record check at the practicum orientation with results acceptable for placement in practicum experiences; delayed results will require dropping the scheduled practicum;
- 3. complete a standard drug screen with results acceptable for placement in practicum experiences;
- 4. purchase student liability insurance;
- 5. provide proof of or purchase personal health insurance; and
- 6. purchase the required student uniform.

- 3. Eligibility for Clinical Placement Information Sheet
- 4. Copy of high school transcript verifying graduation or copy of GED certificate. Coursework must be evaluated by the Southern State Dean of Student Affairs and posted to Southern State transcript to be counted towards prerequisites.
- 5. A printed copy of your Southern State Course History listing, including previously received transcript information, verifying minimum GPA of 2.0, and completion of ENGL 0097 or 0099 and MATH 1116 with a final grade of "C" or higher (review pre-requisite options for ENGL 1101 and MATH 1135). Log into MyRecords, click on "Student", then "My Course Information", select "Unofficial Transcript", scroll to bottom of transcript, click on "Unofficial Transcript PDF" and print (this is the only printout that will be accepted)
Entrants who do not submit all required documents will not be considered. You must schedule an appointment with the Allied Health Office to personally deliver your completed program information packet or review option for e-submission. On your request, when hand delivering your documents to the Allied Health Office, the content of your information packet will be reviewed to identify that all required documentation is included.
Note: A felony or misdemeanor may disqualify a candidate from being certified by the State of Ohio (as outlined in Section 4761.04 of the Ohio Revised Code).
Core Education and Open Enrollment MAST/ALTH courses may be completed prior to entry into the program or during the appropriate semester of the medical assisting program.
Current knowledge of prerequisites is expected. Refer to the college Catalog for any additional requirements of the below courses.
Step 1 - Pathway to Phlebotomy Technician
After successful completion of Step 1, you will be eligible to earn the Phlebotomy Technician (PBT) credential through the American Society of Clinical Pathology (ASCP), the Registered Phlebotomy Technician (RPT) credential through American Medical Technologists (AMT) and/or the Certified Phlebotomy Technician (CPT) credential through National Healthcareer Association (NHA).

Goals:
- To maintain a Phlebotomy Technician program that is up-to-date with the changing health care field.
- To prepare competent entry-level phlebotomy technicians in the cognitive (knowledge), psychomotor (skills), and affective (behavior) learning domains.
- To work with students, college, and community to provide a program that meets the needs of health care and the communities of interest.
- To maintain a Phlebotomy Technician program consistent with Ohio statues and regulations for the college and specified guidelines provided by agencies offering credentialing examinations for phlebotomists.
Objectives:
- Review literature, publications, and attend seminars that are health-related to maintain awareness of changes in the health care field and instructional/educational changes.
- Review and revise the Phlebotomy Technician curriculum on a regular yearly basis.
- Review and revise the Phlebotomy Technician program course syllabi on a regular yearly basis.
- Maintain any needed and/or scheduled meetings of the Phlebotomy Technician Advisory Committee to gain information on local health care in order to incorporate this into the curriculum.
- Survey practicum sites to gain input into the strengths and/or weaknesses of the Phlebotomy Technician program and incorporate this information into any Advisory Committee and into curriculum review.
- Maintain an active professional development program to keep staff abreast in current health care and in education/instruction.
- Encourage participation in student organizations, professional phlebotomy technician organizations and in community projects that are health related.
